Mid-Atlantic Path Forward Collaborative Care Webinar Session
The need for timely, affordable and effective care for mental health and substance use disorders has never been greater, and the Collaborative Care Model (CoCM) offers an opportunity to materially address this need in the near term. A proven model for integrating mental health and substance use care into primary care, CoCM’s patient-centered approach has been shown to improve access and clinical outcomes in a solid evidence base that includes more than 80 randomized trials.
The MidAtlantic Path Forward (MAPF) is continuing its series of monthly 1-hour webinars showcasing CoCM programs that have been successfully-implemented in small primary care practices as well as large multi-site health systems. All sessions are held from 12-1pm ET on Fridays and will be posted on the MAPF website.
Each session features two presentations highlighting different practice/patient profiles, scope, payer mix, and geographies. The focus is on the practicalities of implementing and sustaining CoCM programs: startup and funding; leadership buy-in and support; CoCM workflows, including billing; training; psychiatric consultants; implementation barriers and solutions, and patient outcomes.

- Patricia deSa, Director of the National Mental Health, Wellness, and Addiction Care program at Kaiser Permanente, describing how the organization moved from a small pilot to large scale implementation of CoCM with a focus on equity, performance improvement, and fidelity to the evidence-based model.
- Dr. Hani Talebi, Chief Clinical Officer at the Meadows Mental Health Policy Institute, will discuss facilitators and barriers the Meadows Institute experienced while collaborating with Texas A&M University to implement one of the first full-fidelity CoCM programs at a University Health Center in the state of Texas, highlighting the unique aspects of the CoCM for transitional-aged youth and young adults.
